Download the Adobe Acrobat Reader , (free software) in order to read the Acrobat PDF formatted files. Look for Acrobat on your system before downloading the application because many personal computers are sold with Acrobat preinstalled.

Instructions

1. Print the Membership Form (two formats are provided)
   • Download the Adobe Acrobat version
   • Download the Microsoft word version
   If you are unable to print a copy of the form then please use the online Membership Form Request and we will use the US Mail to send you a paper version of same.

2. Fill in the printed Membership Form.

3. Kyu ranks (non-black belts) must have your instructor's signature to verify stated rank.

4. Mail the signed form along with a check, made payable to the American Judo and Jujitsu Federation, to the AJJF Central Office.
